Driving Licence Documents and Fees: Full List and Form 1A
For most licence services you need proof of age, proof of address, a photo and signature, and a fitness form: Form 1 (your own declaration) or Form 1A (a doctor's certificate, needed for a transport licence and for renewal from age 40). The central fees are set by rule 32 of the Central Motor Vehicles Rules, last revised on 29 December 2016: for example ₹150 per class for a learner licence, ₹300 per class for the driving test and ₹200 for the licence, plus ₹200 for the smart card. States add their own charges, and Sarathi shows the total before you pay.

Which documents do you need for each service?
All these services use the national application Form 2, which Sarathi fills in for you as you go. Choose your service below. Your state can ask for a little more, so follow what the Sarathi page for your state asks you to upload.
Learner licence
- Proof of age and proof of address (see the list).
- A recent photo and your signature (Delhi asks for two photographs).
- Form 1 self-declaration of fitness, filled in online.
- Form 1A medical certificate for a transport vehicle. Delhi and Tamil Nadu also ask for it from age 40 for other classes.
- Under 18 (a gearless two-wheeler up to 50 cc from 16): a declaration from your parent or guardian.
Full steps: learner licence guide.
Permanent driving licence (driving test)
- Your valid learner licence (the original, in Delhi).
- A recent photo.
- For the test vehicle, Tamil Nadu lists its RC, insurance, tax card and PUC certificate.
- If you trained at a driving school: its Form 5 certificate. A Form 5B from an accredited driver training centre exempts you from the RTO test; an ordinary Form 5 does not.
- For a transport licence: Form 1A, and in Delhi Forms 5, 14 and 15.
Full steps: permanent licence guide.
Renewal
- Your licence number and date of birth (the record is fetched from the national register).
- Form 1 if you are under 40 with a private (non-transport) licence.
- Form 1A if you are 40 or older, or hold a transport licence.
- Proof of your new address, only if you are also changing it.
Full steps and validity by age: renewal guide.
Duplicate (lost, stolen or damaged)
- A copy of the police complaint if the licence was lost, stolen, mutilated or damaged (Form 2 asks for it; Delhi asks for an FIR or NCR with the licence number).
- The damaged card, or a copy of the licence if you have one.
- Form 1A only if you are also renewing at 40 or older, or hold a transport licence.
Full steps: duplicate licence guide.
Change of address or name
- Address: one proof of the new address from the same list.
- Name: Form 2 lists an affidavit sworn before a First Class Judicial Magistrate or a notary, a marriage certificate, and a copy of a newspaper advertisement. Ask your RTO which of these it needs for your case.
Full steps, including date of birth and photo: change or correct licence details.
International Driving Permit
- Form 4A, filled in online, with a copy of your valid Indian licence.
- Passport-size photographs and proof of your place and date of birth.
- Your passport (Form 2 also mentions a visa; the 2021 amendment dropped the visa and medical conditions, but some state pages still list them).
Full steps: IDP guide.
With voluntary Aadhaar authentication, many of these services can be done online without visiting the RTO. Without Aadhaar you still apply online, then take your documents to the RTO.
Which documents are accepted as proof of age and address?
Form 2 accepts any one of these as proof of address and age. If the same document shows both your date of birth and your current address, one document is enough; otherwise give one of each.
- Aadhaar
- Passport
- Electoral roll entry
- Life insurance policy
- School certificate
- Birth certificate
- Pay slip from a central or state government office or a local body
- Affidavit sworn before an Executive Magistrate, a Notary Public or a First Class Judicial Magistrate
- Certificate of age from a registered medical practitioner not below the rank of Civil Surgeon
- Any other document your state government specifies (see your state's Sarathi page)
Check that the copy you upload actually prints the detail you are proving, and that it is readable. Foreign nationals also need proof of legal presence in India, along with proof of residence.
What are the central licence fees (rule 32)?
Rule 32 of the Central Motor Vehicles Rules, 1989 sets these fees across India. The amounts below are as revised by the Ministry of Road Transport and Highways notification of 29 December 2016, as published by the transport departments of Uttarakhand, Himachal Pradesh and Andhra Pradesh. Each service has its own rule number; the sources below list them.
| Service | Fee |
|---|---|
| Learner licence, for each class of vehicle | ₹150 |
| Learner licence test, or repeat test | ₹50 |
| Driving test (test of competence), or repeat test, for each class | ₹300 |
| Issue of a driving licence | ₹200 |
| Smart card licence (Form 7), on top of the service fee | ₹200 |
| Adding another class of vehicle | ₹500 |
| Renewal of a driving licence | ₹200 |
| Renewal "after the grace period" | ₹300 + ₹1,000 for each year or part of a year of delay |
| Change of address or other particulars | ₹200 |
| International Driving Permit | ₹1,000 |
| Endorsement or renewal of authorisation to drive vehicles carrying hazardous goods | ₹100 |
| Appeal against an order of the licensing authority | ₹500 |
| Driving school licence, issue or renewal | ₹10,000 |
| Duplicate driving school licence | ₹5,000 |
The table still uses the words "after the grace period" from 2016. Since 2019 the law treats a licence as invalid from the day after it expires, so the exact late fee depends on how long ago it expired. Sarathi works it out; see expired licence.
Paid together: the learner licence fee, its test fee and the driving test fee (the first three rows) are paid at the same time when you apply for a learner licence, a driving licence or another class.
What will you actually pay in your state?
States add user charges, service charges and postage, and bundle the fees differently. Two worked examples from state transport websites:
Delhi
| Stage | Total |
|---|---|
| Learner licence, 1 class ₹150 + ₹50 + ₹300 | ₹500 |
| Learner licence, 2 classes ₹300 + ₹50 + ₹600 | ₹950 |
| Learner licence, 3 classes ₹450 + ₹50 + ₹900 | ₹1,400 |
| Learner licence retest | ₹50 |
| Permanent licence ₹200 + ₹200 smart card | ₹400 |
So in Delhi the ₹300 driving test fee for each class is paid up front with the learner licence, and only ₹400 is left at the licence stage. A repeat driving test costs ₹300 again.
Tamil Nadu
| Stage | One class | Two classes |
|---|---|---|
| Learner licence | ₹230 | ₹380 |
| Driving licence | ₹800 | ₹1,100 |
Tamil Nadu publishes totals only, which include its own charges on top of the rule 32 fees.
For any other state, Sarathi shows the full amount on the payment page before you pay. The doctor's charge for a Form 1A is not a government fee; ask the doctor.
Form 1 or Form 1A: which medical form do you need?
Form 1: your own declaration
You answer questions about your own fitness: epilepsy or fits, eyesight (whether you can read a car number plate at 25 metres), use of your arms and legs, colour vision, night blindness and hearing. No doctor signs it. You fill it in online as part of the application.
Who uses it: private (non-transport) applicants under 40.
Form 1A: medical certificate
A registered medical practitioner examines you and certifies your eyesight, colour perception, hearing, night vision and any condition that could affect driving. The form also has space for your blood group.
- Valid for 6 months (rule 5(3)).
- Sarathi has a Find Doctor link to find doctors registered for this.
- In some states the doctor issues it online on Sarathi. Odisha did this from 10 March 2022, with MBBS doctors.
Who needs which form?
- Any transport (commercial) licence: learner, new, added class or renewal: Form 1A
- Renewal at age 40 or older: Form 1A
- Learner licence at 40 or older, private classes: Form 1A in Delhi and Tamil Nadu; otherwise as your state asks
- Adding a class to an existing licence: Form 1A in Odisha; elsewhere ask your RTO
- Anything else, under 40, private: Form 1
- International Driving Permit: none under the 2021 rules
